Proven Strategy to Reduce Hospitalizations

There is ONE simple strategy that can reduce emergent care and hospitalizations for our patients: therapists and nurses need to check the medications on every visit. Medication reconciliation, education and management are BASIC to home health.
 

If we don't ask these questions on EVERY visit, we cannot ensure that the patient is taking their medication appropriately nor can we assess the effectiveness of the medication!

Here is a sample script:

     Have you seen the doctor since my last visit?

     Has your doctor ordered any new medications?

     Has your doctor changed the frequency of any of your medications?

     Has your doctor changed the dose of any of your medications?

     Has your doctor discontinued any of your medications?

     Have you bought any non-prescription medications?

     Have YOU changed anything about how you are taking your medications?

 If you are not asking these questions at every visit you need to.
