Evaluations of medical eligibility for Medicaid covered in-home, long-term care services can be conducted by staff nurses employed by Pyramid Home Health Services as well as State Department of Health and Senior Services case managers and other state contractors. The purpose of the assessment is to identify and evaluate those un-met ADL/ IADL needs which may cause certain individuals to require Medicaid reimbursed in-home services in lieu of nursing home placement.
